Formula

  1. Deductible Applied = min(Procedure Cost, Remaining Deductible)
  2. Insurance Would Pay = (Procedure Cost - Deductible Applied) x Coverage%
  3. Insurance Pays = min(Insurance Would Pay, Remaining Annual Maximum)
  4. Out of Pocket = Procedure Cost - Insurance Pays

Example

$2,000 procedure, $50 deductible remaining, 50% coverage after deductible, $1,500 annual maximum remaining:

Deductible applied: $50 → Insurance would pay: $975 (under the $1,500 cap) → Out of pocket: $1,025

Why the Annual Maximum Matters Most
