The Heartbeat Is an Electrical Event

Every heartbeat is triggered and coordinated by electrical activity spreading through the heart muscle. An electrical wave causes the heart's main pumping chambers to contract, and then those chambers must electrically recover, or reset, before they can be activated again for the next beat. This cycle of electrical activation followed by recovery is what an electrocardiogram traces. The QT interval measures the span from the start of the chambers' electrical activation through the completion of their recovery, essentially the time the heart's electrical system spends firing and then resetting itself.

Why Heart Rate Complicates It

Here is the difficulty the correction addresses: the QT interval naturally changes with heart rate. When the heart beats quickly, the whole cycle, including the recovery phase, compresses, and the QT interval shortens; when the heart beats slowly, it lengthens. This is normal physiology, the electrical timing adapting to the pace of the beats. But it means the raw QT interval cannot be judged against a fixed standard without knowing the heart rate, because a perfectly normal interval at a fast rate would look different from a normal one at a slow rate. Rate confounds the measurement.

Correcting to a Common Rate

The correction solves this by adjusting the measured QT to estimate what it would be at a standard reference heart rate, so intervals measured at different rates can be compared on equal footing. A formula from early in the twentieth century does this adjustment, and while it is known to over-correct at heart rates far from the reference, prompting the use of alternative formulas in extreme cases, it remains widely used.
