Cardiovascular risk is not driven by any single measurement. It emerges from the interaction of several, which is why a person with modestly elevated blood pressure, borderline cholesterol and a smoking habit can carry more risk than someone with one dramatically abnormal value and nothing else. This calculator tallies six commonly cited risk factors into a simple point score, purely to make that interaction visible — it is an educational illustration of how the factors relate, not a validated risk equation.

The Formula
0 to 3 → Low estimated risk
4 to 6 → Moderate estimated risk
7 or more → High estimated risk
The theoretical maximum is 12 points. The bands are educational groupings, not clinical thresholds, and the score deliberately produces no percentage — putting a number like "14% ten-year risk" on this arithmetic would imply a precision it does not have.
What the Point Weights Say
Read the table as a statement about relative contribution and something becomes apparent: smoking and diabetes each carry the same two points as severely elevated cholesterol or blood pressure, and only age can contribute three. Age is the factor nobody can modify and it carries the most weight — which is precisely the argument for taking the five that are modifiable seriously. Two of them, smoking and diabetes, are binary in this scheme: there is no partial credit, and the entire two points vanish or appear at once.
Where This Kind of Tallying Is Used
- Health education — point systems make the multiplicative nature of risk factors intuitive in a way that a list of numbers does not.
- Prompting a conversation — seeing several factors stack up is often what motivates someone to raise the subject with their doctor.
- Understanding the real tools — validated instruments such as ASCVD and QRISK work on the same principle, with far more rigour behind the weights.
- Illustrating modifiability — toggling the smoking input and watching two points disappear makes a point that no amount of prose does.

These are round-number simplifications of continuous relationships. Real risk does not jump at a birthday or at a single mg/dL, and no validated model treats it as though it does.
How to Use This Calculator

- Enter Total Cholesterol (mg/dL) from a lipid panel. To convert from mmol/L, multiply by 38.67.
- Enter Systolic Blood Pressure (mmHg) — the upper of the two numbers in a blood pressure reading.

The sex- and race-specific multivariable Cox proportional hazards regression model integrates nine clinical parameters:
- Age: Evaluated between 40 and 79 years.
- Total Cholesterol & HDL Cholesterol (mg/dL): Measures atherogenic lipid particle burden vs. protective reverse cholesterol transport.
- Systolic Blood Pressure (SBP, mmHg) & Treatment Status: Measures arterial wall shear stress and endothelial microvascular strain.
- Diabetes Mellitus Status: Accelerates coronary vascular atherosclerosis and plaque calcification.
- Current Tobacco Smoking Status: Promotes platelet aggregation, endothelial dysfunction, and coronary vasospasm.
Cardiovascular Risk Enhancers
For patients in borderline or intermediate risk categories, cardiologists evaluate secondary risk enhancers: Family history of premature ASCVD (males < 55, females < 65), High-Sensitivity C-Reactive Protein (hs-CRP ≥ 2.0 mg/L), Lipoprotein(a) ≥ 50 mg/dL, and Coronary Artery Calcium (CAC) Agatston Score > 100.

Coronary Artery Calcium (CAC) Agatston Zero Score
In middle-aged patients with intermediate ASCVD risk, a CAC Score of Exactly Zero (CAC = 0) demonstrates the absence of calcified coronary plaque, allowing clinician and patient to safely defer statin initiation for 5 years.
