Why low albumin can make calcium look falsely low
Roughly 40% of blood calcium is bound to albumin - when albumin is low, as is common in malnutrition or chronic illness, total measured calcium can appear low even when physiologically active (ionized) calcium is completely normal.
Worked example
For a measured calcium of 8.0 mg/dL with an albumin level of 2.5 g/dL:
Corrected Calcium = 8.0 + 0.8 x (4.0 - 2.5) = 9.2 mg/dL
This correction estimates what total calcium would read if albumin were at the reference value of 4.0 g/dL. When precision genuinely matters clinically, a directly measured ionized calcium is more reliable than any albumin-based correction formula, since the correction is itself an approximation.
