The Per-Session Price Hides the Real Commitment
Therapy is almost always quoted per session, which is the least useful unit for deciding whether you can afford it. A per-session figure feels manageable in isolation; the same figure multiplied by weekly attendance across a year is a different conversation entirely, and it's the one your budget actually has to answer. This calculator converts a session price, a frequency, and an insurance percentage into monthly and annual out-of-pocket totals.
The Formula
Covered Amount = Gross Monthly Cost × (Insurance Coverage % ÷ 100)
Out-of-Pocket Monthly = Gross Monthly Cost − Covered Amount
Out-of-Pocket Annual = Out-of-Pocket Monthly × 12
The insurance model here is a flat percentage of the gross cost. Real plans are frequently more complicated — deductibles that must be met before coverage begins, fixed copays rather than percentages, session caps per calendar year, and out-of-network rates that differ from in-network ones. This calculator deliberately does not model any of that, which is precisely why the output is an estimate for planning rather than a prediction of your bill.
How Session Price Scales
At four sessions a month with no insurance coverage, every figure computed from the formula above:
| Cost per session | Gross monthly | Out-of-pocket monthly | Out-of-pocket annual |
|---|---|---|---|
| $60 | $240.00 | $240.00 | $2,880.00 |
| $80 | $320.00 | $320.00 | $3,840.00 |
| $100 | $400.00 | $400.00 | $4,800.00 |
| $150 | $600.00 | $600.00 | $7,200.00 |
| $200 | $800.00 | $800.00 | $9,600.00 |
| $250 | $1,000.00 | $1,000.00 | $12,000.00 |
These session prices are illustrative inputs chosen to span a broad range, not a claim about typical market rates — therapy pricing varies too widely by region, modality, and provider for any single figure to be meaningful. Use your own quoted rate.
What Coverage Is Worth
Insurance percentage moves the annual figure more than most people expect. At four sessions a month and $100 per session:
| Coverage | Covered monthly | Out-of-pocket monthly | Out-of-pocket annual |
|---|---|---|---|
| 0% | $0.00 | $400.00 | $4,800.00 |
| 20% | $80.00 | $320.00 | $3,840.00 |
| 50% | $200.00 | $200.00 | $2,400.00 |
| 70% | $280.00 | $120.00 | $1,440.00 |
| 80% | $320.00 | $80.00 | $960.00 |
| 100% | $400.00 | $0.00 | $0.00 |
Computed from this calculator's formula. The jump from 0% to 80% coverage is the difference between $4,800 and $960 a year — which is why confirming your actual coverage percentage before assuming therapy is unaffordable is worth the phone call.
Frequency Is the Other Lever
At $150 per session with no coverage, attendance frequency alone spans a wide range:
| Pattern | Sessions/month | Out-of-pocket monthly | Out-of-pocket annual |
|---|---|---|---|
| Monthly | 1 | $150.00 | $1,800.00 |
| Fortnightly | 2 | $300.00 | $3,600.00 |
| Weekly | 4 | $600.00 | $7,200.00 |
Weekly attendance is treated as 4 sessions per month here. Over a full year weekly sessions actually average closer to 4.33 per month, so a strictly weekly schedule would run somewhat above the annual figure shown — this calculator multiplies your stated monthly figure by 12 rather than modelling the calendar.
Where This Is Genuinely Useful
- Budgeting before the first appointment — knowing the annual figure in advance prevents the more painful discovery three months in.
- Comparing two providers — a cheaper therapist who is out-of-network can easily cost more than a pricier in-network one; running both through the calculator settles it.
- Weighing frequency honestly — if weekly isn't affordable, seeing the fortnightly figure makes a sustainable schedule easier to commit to than dropping the idea entirely.
- Preparing questions for your insurer — the gap between coverage scenarios shows exactly why "what percentage, and is there a deductible or session cap?" is the call worth making.
How to Use This Calculator
- Enter Sessions per month — use 4 for roughly weekly, 2 for fortnightly, 1 for monthly. Must be greater than zero.
- Enter Cost per session ($) — your provider's actual quoted rate rather than an estimate, if you have it.
- Enter Insurance coverage (%, 0 if none) — the percentage your plan reimburses. Enter 0 if paying entirely out of pocket. Accepts 0 to 100.
- Select Calculate to see the gross monthly cost, the covered amount, your monthly out-of-pocket figure, and the estimated annual total.
If your plan has a deductible you haven't yet met, running the calculation twice — once at 0% and once at your post-deductible percentage — brackets your likely year more realistically than either figure alone.
